Waiver and Release of Liability

I voluntarily register and participate in the TAKBUHIN – ADHIKAIN – LAYUNIN: “Bawat Hakbang Mahalaga” Fun Run (“Event”).

I acknowledge that running and participating in outdoor athletic activities involve inherent risks, including but not limited to falls, collisions, adverse weather conditions, road hazards, dehydration, heat-related illnesses, accidents involving vehicles or other participants, and other unforeseen circumstances that may result in serious injury, permanent disability, or death.

Knowing these risks, I voluntarily assume full responsibility for my participation.

I hereby release, discharge, and hold harmless the Organizing Committee, Apolinario Mabini Masonic Lodge No. 235, its officers, members, volunteers, sponsors, partners, event officials, marshals, medical personnel, government agencies providing assistance, venue owners, and all persons connected with the Event from any and all claims, liabilities, demands, damages, actions, causes of action, costs, and expenses arising out of or relating to any injury, illness, loss, damage, accident, or death that may occur before, during, or after the Event, except where caused by willful misconduct or gross negligence as provided by applicable law.

I agree to abide by all race rules, safety regulations, and instructions issued by race officials. I understand that the organizers reserve the right to modify, postpone, suspend, or cancel the Event due to weather conditions, force majeure, public safety concerns, or other circumstances beyond their reasonable control.

Medical Declaration

I certify that:

• I am physically fit and medically capable of participating in this Event.
• I have consulted a physician when appropriate regarding my participation.
• I am not suffering from any medical condition that would make participation unsafe.
• I will immediately stop participating and notify race officials if I experience chest pain, dizziness, difficulty breathing, severe fatigue, or any medical emergency during the Event.

I authorize the Event’s medical personnel to administer first aid and arrange emergency medical treatment or transportation when deemed necessary. I understand that any medical, hospital, ambulance, medication, or related expenses shall be my sole responsibility.

Data Privacy Consent

In accordance with the Data Privacy Act of 2012 (Republic Act No. 10173), I voluntarily consent to the collection, recording, storage, processing, and use of my personal information by the organizers for legitimate purposes related to this Event.

My personal information may be used for:

• Event registration and participant verification;
• Race kit preparation and bib assignment;
• Event communication and announcements;
• Emergency response and participant safety;
• Publication of official race results;
• Event documentation and reporting;
• Future communications regarding similar activities organized by Apolinario Mabini Masonic Lodge No. 235, unless I withdraw my consent.

The organizers shall implement reasonable security measures to protect my personal information and shall not disclose it to unauthorized third parties except when required by law or necessary for the conduct of the Event.

Photo and Video Consent

I grant the organizers and their authorized representatives permission to photograph, video record, and otherwise capture my likeness during the Event.

I authorize the use of these photographs, videos, recordings, and my name for legitimate documentation, publicity, historical records, promotional materials, social media, print publications, and other non-commercial communications related to the Event without additional compensation or prior notice.

Participant’s Declaration

I certify that all information I have provided is true and correct.

I acknowledge that I have carefully read and fully understood this Waiver, Release of Liability, Medical Declaration, and Data Privacy Consent.
